Dear Mr. Hernandez:

I write to file a Report and Complaint of harassment by my supervisor, Lloyd Barron.

Events: On Tuesday, May 3, 2008 Lloyd touched me, against my wishes, and did not stop doing so even after I told him to stop. On May 24, 2008, Lloyd asked me to go out with him on a date. I declined. He has asked me to go out on a date with him 4 times since then, each time more insistently, although each time I told him “No, and please don’t ask me again.” Since that time, Lloyd has yelled at me several times, given me large assignments to complete in an unreasonably short time, and has threatened to demote me.

Witnesses: Caroline Kravitz and Martin Fledsoe have seen several of these events, as has Pat Cronin and George Tarlow.

Written Materials: I have seven emails in which Lloyd exhibited his harassment and hostility. I will deliver them to you at my earliest opportunity.

Requests: Please (a) immediately separate me from Lloyd Barron, (b) initiate an investigation of this behavior, and (c) ensure that neither Lloyd nor any of his friends and colleagues retaliates against me for filing this Report and Complaint with you.

Confidentiality: I ask that this matter be handled in as confidential a manner as is possible.

Other Steps: If there are any other steps or measures that I should take in order to ensure that this matter is treated with the utmost seriousness, please let me know what they might be, preferably by email.

Please acknowledge: Please acknowledge that you have received this Report and Complaint by simply sending me a “Reply” email with the words “Report/Complaint Received.”
